The following on-line documents are provided to facilitate newly hired MCPS substitute paperwork. New substitutes should access, complete, and print copies of the documents listed below and bring these documents to the Human Resources Department at your scheduled appointment time. Our Human Resources staff will review your paperwork for completion and take your fingerprints.
Alcohol and Drug-Free Workplace Acknowledgment
Child Protective Services Form (form currently unavailable online)- Please print this form front and back so that the entire form is on one sheet of paper (do not include the instruction page). Forms not printed front and back cannot be accepted. If you are unable to print the form front and back, we can provide a copy for you. Do not sign this form until you are in the presence of a Notary Public. MCPS Human Resources staff can notarize the form for you. You must bring a money order, certified check, or cashiers check for $10 made payable to Montgomery County Public Schools. We cannot accept personal checks, debit/credit cards, or cash.

Fingerprint Form - Fingerprints are done at the MCPS School Board Office by the Human Resources staff. We cannot accept background records from other agencies. You must bring a money order, certified check, or cashiers check for $27 made payable to MCPS. We cannot accept personal checks, debit/credit cards, or cash.

I9 Employment Eligibility Verification - This form is fillable. However, only Section 1 will be completed by you. When you have completed section 1 and are ready to print, you may receive a message that there are incomplete areas. You can answer "yes" to continue to the print option.
TB Test Physician Form- If you have had a negative TB test done within the past 12 months, you may bring a signed certification from your health care provider in lieu of this form. The certification must indicate the date the test was performed and read.
Employees who have resided in another state within the last five years may be required to complete a Child Protective Services form for that state. MCPS Human Resources will inform you if this is needed.
